**Job overview**:
As a Senior Support Worker, you will be an active and positive member of the adult learning disability service, contributing skills and experience to the clinical assessments and interventions.

The intensive support team work in collaboration with its community colleagues to ensure we deliver a multi disciplinary team approach (MDT) to the care needs of our clients. The MDT consists of consultant psychiatry, nursing, occupational therapy, speech and language therapy, psychology, dietitian, physiotherapists, support worker and administrative staff.

Our aim is to have a whole team approach. We will work to support the whole person and their family.

This is a county wide service of which the support worker role is integral to offering care/interventions to the clients that we serve.

The team has a base in the City of Truro which is fairly central within the County of Cornwall. Cornwall has many breathtakingly beautiful beaches and has one of the most stunning coastlines in the UK. It also has layers of artistic, educational and culture opportunities.

**Main duties of the job**:
We are seeking an innovative/creative individual to work as part of the Cornish adult intensive support team working with adult clients who have a learning disability

To undertake behavioural assessment and intervention under the guidance of a qualified practitioner. Working with the individuals with a learning disability and their families you will monitor, contribute to, and modify the person-centred care plan in response to need, in order to improve the health outcomes for the person.
