Supportive Outreach and Engagement Worker (Alcohol Care Teams)

9-month Fixed term/ secondment contract to be based under the Alcohol Care Teams across Manchester Foundation Trust till March 2025

A new opportunity has arisen to join the MFT Alcohol Care Team (ACT), This is an exciting time to join the team, as it expands and develops within the hospitals, and the wider trust.

x2 Supportive outreach and engagement worker positions had been created from additional funding, to support individuals accessing secondary emergency care services, providing a wider, holistic and multi-agency approach to case management.

The focus of the role would be working with high intensity users of the trust, who present to the Emergency Departments or are admitted in relation to alcohol misuse. Ensuring effective implementation of care pathways and being the interface between community substance misuse services and the Alcohol Care Teams. Providing the extension of care beyond discharge, to support individuals who may be resistant to services, hard to engage, or treatment naïve at point of discharge. The post holder will attempt to re-engage individuals who have dropped out of treatment or detached from services

The post holder will develop hospital Multi-disciplinary team meetings for high intensity users and attending relevant complex planning meetings for these individuals with multiple needs.

**Professional Duties and Responsibilities**:
To work independently as a key member of the Alcohol Care Team

Accepts referrals from MFT Alcohol Care Teams, identified as high intensity users

Carry a defined case load of service users with complex needs who frequent the Trust due to alcohol related harm

Manage complex cases, in liaison with the medical team/ ward staff/ Lead Alcohol Nurse Specialist/ community alcohol services / community services

Optimise the continuity of care for patients by developing active communication links across several relevant agencies, including Acute Trust, Community based substance
misuse services, Primary Care, Social Care, and Third Sector agencies

Assist in providing strong links between ACT and community settings

Liaise with specialist and community-based services, to ensure those patients and their families or carers, have access to appropriate services
