We have an exciting opportunity within Central London Community Health (CLCH) for anyone interested in looking for a new challenge, learning about a whole new area of medicine. This is an opportunity to be part of new services and pathways for patients with post covid syndrome (Long Covid) and help continue to develop new clinical expertise. These are patients who are experiencing a range of symptoms over 12 weeks post covid infection.

This is a new Post Covid MDT for the inner London boroughs of K&C, Westminster and H&F part of CLCH. This small team will consist of an Occupational Therapist, Physiotherapist, Psychologist and Rehabilitation Assistant (of varying grades), who will work virtually to provide assessment, intervention, support and case management for this cohort in the community. The team will link in with the acute hospital assessment clinics, community providers, and community and voluntary sector resources. They will help the patient access interventions and education from different sources and manage the patients through the Living Well post covid app and other virtual group interventions.

We are looking to recruit a Full-Time Band 4 Assistant Practitioner. We are open to flexible hours, working from home, job shares and split posts-this can be discussed at interview.

As an Assistant Practitioner within the Post-Covid Recovery Service you will:

- Work under the supervision of a qualified Allied Health Professional (AHP) to support service users, their families, friends and carers, and play an important role in how they experience our service.
- Work independently to follow a treatment plan and be motivated to deliver the patient’s treatment goals to a high standard.
- Demonstrate good communication with the service users, team, other health and social care professionals and a range of outside agencies.
- A significant part of the role includes using digital technology eg electronic patient records, Living With Covid app, so you will need to have good IT literacy skills.
- The role also includes administrative responsibilities such as patient bookings, creating letters. As well as organisation of patient interventions eg group activities and others.
- You may be required to participate in the rotation/deployment of staff across Post Covid services across NWL to as required to provide cover, mutual aid or mitigation against increasing demand.
- The service operates between the hours of 9.00-17.00
