**What will you do?**

The Primary Care Team work with service users registered at GP practices across the county, supporting them to achieve their goals and to find the right support for their needs and desires. We are a fun, caring and dynamic team which places a high value on mutual support, sharing our wide-ranging knowledge and expertise and maintaining close working relationships, despite working in different areas of the county. There are lots of opportunities for staff to develop areas of interest when it comes to working with people; on an individual and team basis. We are truly committed to the wellbeing of our staff and this remains a high priority alongside providing excellent support for our service users.

We are looking for someone who is motivated, pro-active and supportive, with a strong commitment to person-centred work and service user involvement and is interested in being a part of a fast growing team of Wellbeing Workers from all walks of life. We provide a mixed model of face to face and telephone work, integrated within GP practice teams.

Key responsibilities*

The key responsibility of the Primary Care Wellbeing Worker in this role is to provide an excellent person-centred service via wellbeing sessions.
- Provide initial assessment sessions and follow-up sessions as required.
- Work with individuals to identify their own hopes and goals, and support them to develop a personalised action plan to increase resilience and wellbeing.
- Review and assess progress of individuals, supporting them to complete their personalised action plans.
- Carry out follow up contact with individuals to evaluate outcomes using designated measures such as the Short Warwickshire-Edinburgh Mental Wellbeing Scale (SWEMWBS) and patient satisfaction questions.
- Telephone appointments - we operate a mixed model of telephone work and face to face work (carried out in GP practices).
- Collect and accurately record service user data onto an online database.
- Keep up to date with appropriate information about the wider Oxfordshire Mind Wellbeing Service - and other local, regional and national services - to ensure service users can be offered appropriate information and signposting.
- Provide advice and guidance to GP practice staff about community and voluntary sector services.
- Forge and maintain excellent relationships with Primary Care staff to help ensure the service is well understood, and that staff are aware of ongoing work with service users.
- Promote excellent partnership working within GP practices including with all primary care staff, other practice link workers and other key stakeholders.
- Liaise with practice staff to ensure the service remains accessible to all individuals and groups e.g. younger and older adults, women with children, black and minority ethnic communities, veterans and groups who may have difficulty in accessing services.
- Work flexibly as part of the wider Wellbeing Service including attending team meetings as required.
