This is an amazing opportunity to join our new Care Navigation service within Brownsover Medical Centre, Rugby.

This team is the driving force behind our mission for patients, to deliver a service where they can quickly and easily receive the right care from the right person, at the right time, in the right place.

At the forefront of our triage service for the GP practice, you will schedule the majority of appointments for patients across our 4 Warwickshire practices.

You will:

- Working collaboratively across a multi-disciplinary team and external healthcare professionals.
- Deliver patient centred support to patients to access the care they need.
- Ensuring appointments are triaged effectively through digitised systems including EMIS.
- Provide administrative support to the practice and clinicians to ensure patients care is processed correctly and efficiently.
- Where appropriate, signposting patients to alternative care and/or provide self-management advice and support.
- Contributing effectively to overall KPI targets.

**Experience**:

- Primary care: 1 year (preferred)
- EMIS: 1 year (preferred)
- triaging: 1 year (preferred)
