JOB PURPOSE/SUMMARY The jobholder will be actively involved in providing direct and indirect care for patients in support of the multidisciplinary team. The post holder will be expected to work under direct or indirect supervision of the clinical nurse specialists supporting the multidisciplinary team. The jobholder will assist in the overall implementation and evaluation of care as part of the multi professional team. The jobholder is expected to plan, implement, deliver and maintain high standards of patient care.

The jobholder will be skilled in taking appropriate action(s) to achieve the planned outcome. The jobholder will contribute to the maintenance of an environment for effective care The jobholder will provide a single point of contact for the patients/carers and will be expected to work autonomously to provide this service. The jobholder will be expected to practice in accordance within Trust policies, guide lines defined competency boundaries and local l standard operating procedures. The jobholder will work as part of a multidisciplinary team, assisting the CNS team with actions arising from MDT outcome discussions and treatment pathways.

The post holder will support the CNS team. PRINCIPAL DUTIES & AREAS OF RESPONSIBILITY Communication and Coordination: Communicates appropriately and sensitively in different settings, using a range of methods and styles, enhanced by underpinning knowledge and experience Anticipates how people may feel in a given situation, responding with kindness and empathy Acts professionally and appropriately in situations, understanding that there may be boundaries to confidentiality, seeking support and advice. Works with colleagues in other services to facilitate safe and effective transition of care between services. Able to document information in a legible, accurate and professional manner Develops partnership working both within and outside of the Trust in order to support the patient, carers, families and staff.

Demonstrates knowledge of care pathways in order to enable effective response to queries. Carries out clerical and administrative duties required by the role Develops the ability to work across professional boundaries Works with MDT co-ordinators in order to facilitate and support patients transition along the care pathway Clinical Responsibilities Triaging incoming telephone calls, managing onward referrals and sign posting to other services Following up on actions required following patient consultations in clinics, including communication to other organisations within the region, clinicians, and or supporting services Undertake actions arising from MDT outcome discussions either under direction of the CNS or as per protocol Take responsibility as appropriate for inputting information onto the LTHT cancer patient electronic record (PPM). Support audit and research through measuring, collecting and collating data. Develop and use appropriate communication skills to support regular face to face and telephone contact with patients, families, careers and external partners Undertake Holistic Needs Assessments in line with agreed Standard Operating Procedures or Guidelines; carrying out resultant actions and or reporting to the CNS team using a patient partnership approach Undertake phone calls to patients following discharge from hospital to ensure that the patient, family and or carer, as appropriate, is aware of future plans and appropriately refer to CNSs when there are issues that fall outside the remit of this role Escalate concerns with regards a patients physical or psychological needs, or changes in their clinical condition to CNS colleagues within the agreed time frame Understand the patient information pathway and support the patient, family and or carer as appropriate, to access resources where appropriate Undertake patient pathway co-ordination responsibilities as appropriately delegated by CNSs Coordinate and organise appointments and assessments as required within the cancer services or associated services Assist with staff education/service improvement events Acknowledge any limitations of competence in accepting care activities and inform a registered practitioner where appropriate Assist in planning, implementation and evaluation of individual programmes of care for patients Support the delivery of patient, family and carer training and education as necessary, including the use of equipment.

Coordinate the necessary assessments, appointments or investigations to fast track people back into the system if required. Organise and prioritise time effectively undertake duties on different wards, departments or clinical areas as and when required by service demands or patient need. Act in such a way that safeguards the health and wellbeing of children & vulnerable adults at all times.
