**Job description**

An exciting opportunity has become available for a Senior Support Worker to join our team.

The role is to take responsibility for a number of our supported living properties. As part of your role, you will act as the main contact for each house, taking responsibility for its daily running, the service users' welfare and the maintenance of contact between the service user’s family and Forge House Care. You will provide support to the service users and act within the management team to ensure the day to day running of the business.

You will provide support and encouragement that empowers our service users to lead an independent lifestyle. You will work in our service users homes, as well as supporting them with activities and accessing the community. You will provide companionship and positive reinforcement to encourage independence in day-to-day activities, which could include personal care, housework, meal preparation and social integration.

Our head office is based in Chatham Maritime and we cover Maidstone, Medway and Gravesend. Therefore, the successful applicant will hold a full UK Driving Licence and have access to their own vehicle.

This is a full time, permanent position offering 40 hours per week, Monday - Friday. There is an on-call responsibility of one weekend in three.

**Duties Include**:

- Attending hand over meetings on Monday mornings and assist with hand overs throughout the day
- Attending and participating in management team meetings
- Attending and participating in team meetings within the staff teams
- Entering notes into the database regarding staff, clients, incidents, sickness etc.
- Arrange daily appointments for service users within assigned houses ensuring transport and sufficient cover is available
- Support staff with issues that arise within the outreach support team
- Complete monthly staff observations and supervisions
- Attend Care Manager reviews, Occupational Therapist visits, meetings with other professionals etc.
- Work in partnership with the FHC Behavioural Therapist to ensure that staff are following the guidelines that have been put in place
- Contribute to the team rota, including obtaining staff availability, planned leave, and matching skill sets with service user needs and preferences
- Contribute to staff meetings and ensure staff are aware of new procedures and company directions

**Job Requirements**:

- Diploma Level 3 or equivalent experience in Health and Social Care
- Experience in a supervisory role will be an advantage
- Full UK Driving Licence and own vehicle is required
- Core hours will be 9am-5pm, however flexibility is required
