West Hallam, United Kingdom Derbyshire Community Health Services NHS Foundation Trust Full time

We currently have an exciting opportunity for highly motivated and enthusiastic ACPs/Trainee ACPs to work within the Urgent Treatment Centres.

We are a flexible employer, offering roles up to and including full time. Our current UTC sites are Ripley, Ilkeston, Buxton and Whitworth.

Are you an existing or an aspiring Advanced Clinical Practitioner with experience of Urgent Treatment?

A Band 7 trainee position may be offered to an aspiring Advanced Clinical Practitioner who is committed to undertaking an apprenticeship training programme.

If you have a passion for life-long learning, enthusiasm for supporting the development needs of others, excellent communication skills and a commitment to continuous quality improvement we would love to hear from you

If offered a trainee position you will commit to undertaking an apprenticeship programme comprising a 3-year MSc in Advanced Practice and the development of an ePortfolio to demonstrate your capability across the four pillars.

Trainees will commence on AFC Band 7 payscale, and uplifted to Band 8a on successful completion of the MSc, successful submission of their portfolio to a cross-organisational review panel.

The role is initially for the southern UTCs, based in Ilkeston and including Ripley, and helping to form a core team of ACPs across the four units in order to provide a model of seven day service cover, ultimately linking the trainees into a core clinical lead support resource. The GM will be responsible for ensuring that the direction of the service continues.

**As an ACP you will**:
Provide clinical leadership, including direct clinical care, and advice on complex issues
Establish a cohesive team approach for the ACP workforce development
Support the UTC service leads and managers with audit, innovation, and performance management
Lead on training needs discussions, based on audit findings, supervision outputs and individual staff knowledge gaps
Assist in feedback for staff appraisals.
Deputise for the GM where necessary, if not being carried out by other members of staff.

You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.

You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

It is an exciting time to join primary care and community services as the national Ageing Well programme gathers pace.

DCHS is working closely with the Primary Care Networks to transform the care of housebound and vulnerable people across Derbyshire through our Team-Up approach.

We’re seeking ACPs with a special interest in the management of the typical conditions seen within a UTC, to provide day-to-day clinical leadership support to the teams.

To provide leadership support for the clinicians already working in the service, to advance the care of the presenting patients (in line with our 4 pillars approach).

We’re looking for ACPs with a proven track record of working at this level in an urgent care environment and who can support the South Service Lead and Unit managers in safe care delivery.
