Social Care Assessor - Cheshire West & Chester Council (HYBRID WORKING)
Good Morning,
We are looking for a **Social Care Assessor **you will be based at**:CH65 0BE**
To assess/review service users who are discharged from hospital with care to identify alternative or continued support requirements in line with relevant legislation ie Care Act
