Key Responsibilities
  • Conduct person-centred assessments and develop discharge plans that promote independence and well-being.
  • Work collaboratively with multidisciplinary teams, including medics, therapy, nursing, and reablement teams, to ensure seamless transitions from hospital to community settings.
  • Provide guidance and support to colleagues, promoting a culture of continuous learning and improvement.
  • Stay up-to-date with relevant legislation, policy, and frameworks, ensuring compliance and best practice in social care.
  • Participate in multidisciplinary meetings, discharge planning, and best interests meetings to ensure effective communication and collaboration.

Requirements

To be successful in this role, you will need:

  • CQSW/CSS/Diploma/Degree in Social Work or equivalent.
  • Social Work England Registration.
  • Enhanced DBS and Adults Barred List clearance.
  • Statutory and policy base of Social Work.
  • Ability to communicate effectively with customers, carers, and families.
  • Resilience and ability to cope positively with change.
