- Work collaboratively with multidisciplinary teams, including medics, therapy, nursing, and reablement teams, to ensure seamless transitions from hospital to community settings.
- Provide guidance and support to colleagues, promoting a culture of continuous learning and improvement.
- Stay up-to-date with relevant legislation, policy, and frameworks, ensuring compliance and best practice in social care.
- Participate in multidisciplinary meetings, discharge planning, and best interests meetings to ensure effective communication and collaboration.
