Social Care Professional

We are seeking a highly motivated and dedicated Social Care Professional to join our team at Cheshire West and Chester Council. As a key member of our Hospital Discharge team, you will play a critical role in supporting adults to leave hospital and return home safely.
Key Responsibilities:
- Conduct person-centred assessments and develop support plans to meet individual needs.
- Work collaboratively with healthcare professionals, carers, and families to ensure seamless transitions.
- Provide guidance and support to colleagues to promote best practice and continuous improvement.
- Stay up-to-date with relevant legislation, policy, and frameworks to ensure compliance.
Requirements:
- Qualification in Social Work or equivalent.
- Registration with Social Work England.
- Enhanced DBS check and Adults Barred List clearance.
- Strong communication and interpersonal skills.
- Ability to work positively with change and promote positive changes.
