Community Mental Health Practitioner

As a Mental Health Practitioner you will act as a Care Coordinator for a dedicated caseload of patients suffering with Serious Mental Illness with whom you would provide person centred, evidence-based, targeted interventions to aid recovery. You will have a working knowledge of current biopsycho-social models of care and understand how to implement such interventions with your patients to promote optimum health and wellbeing. You will be provided with regular clinical and peer supervision.
The team work collaboratively, you will have the support and opportunity to develop your own skills in evidence-based therapeutic interventions in the psychological therapies as well as advanced nursing.
The Mental Health Practitioner role is a Full Time permanent position, and covers the Bridport, Lyme Regis and surrounding areas. You will need to have a valid driving licence and use of a car as you will be required to review and monitor patients in their own homes and within other community settings.
