This is a new and exciting role to work as part of the elective surgical team, due to a fast turnaround of patients elective surgery can be very busy and this new role will help support the team and patients in safe and timely discharge plans. Part of this role will include liaising and discussing with members of the multidisciplinary team to encourage positive patient outcomes by using excellent communication skills, the successful applicant with work with a rounded knowledges of hospital professionals to support the best patient outcome. Ensuring discharges are safe and effective by making referrals to allied health professionals such as physiotherapy, occupational therapy and social care direct. There are many services at our disposal within the hospital to enhance the care and support patient receive at the time of discharge home and within the NHS we pride ourselves on planning discharges at the point of admission to prevent delays and unexpected hospital stays.

Written and Verbal communication skills are imperative and we are looking for a highly enthusiastic person who is able to work independently as well as part of a team to communicate effectively and make decisions in relation to discharge. At times this role may include attending multi-disciplinary team meetings or discussions with palliative care services as well as undertaking and facilitating complex discharges, in addition to the discharge facilitator role the successful applicant will be expected to undertake general housekeeping tasked and support the ward and management team in the general tidiness of the ward, patient safety initiatives and ensuring areas are stocked up with equipment.
