**Hours**: 37.5, Full-time Monday to Friday (may include weekend working in the future)

The SinglePoint of Access service is a newly developed team within the Hospice. This is an exciting time to join the team as you will be fundamental in supporting the ongoing development and growth of the service. The service will be responsible for receiving all external referrals into the hospice and establishing the need and urgency of the referrals, ensuring that the patient receives the right services at the right time. The team works in partnership with the wider clinical services to ensure patient flow and responsiveness.

The role of the care coordinator is to be a first point of contact for all new referrals received into the hospice via the SinglePoint of Access services (SPA). The role is to work with the clinical assessors and care coordinator to identify the patient’s need and establish the most appropriate pathway of care for the patient.

The role is a development post, and the post holder will work closely with the SinglePoint care coordinator and clinical assessors to understand the wide range of services at the hospice, understand the criteria of each service and help to continuously improve the flow of patients through into services.

The post holder will develop key skills in enabling patients to access the right care at the right time, resolving barriers to accessing care and responding to referrals in a timely way. As the role is a development post, knowledge of palliative and end of life care is desirable, but not essential.

The main responsibilities include:

- Make telephone contact with referrers regarding referrals received, clarifying information received and providing information regarding the outcome of the referral
- Be the first point of contact for patients and referrers of patients referred to the hospice in advance of admission/assessment by a hospice professional, providing non-clinical advice and support as necessary.
- Communicating with patients, health professionals, volunteers and service providers to ensure that all support is provided in a timely manner.
- Establish patient concern, help with non-clinical questions and signpost to other services where necessary.
- To highlight from referral information, any urgency of need with the clinical assessors where appropriate.
- Ensure the patient’s progress through the pathway is proactively managed and that any concerns related to waiting times and further assessment are communicated to the relevant service in a timely manner.

**About you**:
We are looking for a highly motivated and enthusiastic individual with the ability to manage a reprioritise their workload. You will be proactive and be able to work well within a multi-disciplinary service. We are looking for an individual with a keen eye for detail, excellent and compassionate communication skills and be willing to develop and learn new skills for the role. We are also looking for someone who has excellent organisational skills, is engaging with others and is able to work well within a team environment.

Skills and experience required:

- Level 2 qualification in Maths or English.
- Experience of working in a coordination role, clinical administration role or equivalent.
- Highly organised with ability to prioritise own workload and work towards targets.
- Ability to work autonomously and as part of a team.
- Excellent communication skills both written and verbal.
- Administrative and excellent IT skills, including Microsoft Word, and Excel.
- Excellent coordination and organisational skills.
- Experience of providing non clinical telephone support for patients in a health care environment.

- Flexible
- Motivated and proactive
