The Transfer of Care Hub and Home First Team are expanding as part of the development to meet the recommendations of the Department of Health’s ‘Hospital Discharge Service: Policy and Operating Model’. This guidance encourages a Home First and Discharge to Assess culture and reinforces the Home First ethos as best practice.

The Transfer of Care Hub supports an integrated approach to hospital discharge and access to community services across the local health economy. As a Band 4 Technical Instructor you will provide support to the team working towards the vision of ‘Right patient, right place, right time’ improving the hospital to home experience promoting a ‘Home First’ culture.

You will be responsible for assisting with the identification and co-ordination of appropriate discharge pathways for patients, developing integrated working relationships with Health and Social Care colleagues. You will also be required to assist with assessments of patients on the wards and within their own environment to help determine ongoing rehabilitation, care or equipment needs emphasising ‘Home First’ principles,

The post will include weekend working and Bank Holiday cover on a rota basis.

This is an exciting opportunity to join a developing service to ensure patients receive enhanced discharge planning, continuity of care and a seamless transition between care providers beyond the acute hospital environment.
