Modern Matron Band 7

We are seeking a Community Matron to work within the Mid Beds Complex Care/Primary Care Home Team providing long term condition and minor illness assessments for patients that meet the service two-hour response criteria.

You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop individuals and teams to deliver consistently high quality services. You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills. You will be an experienced Registered Nurse with top to toe assessment and independent prescribing skills with a background in minor illness or chronic disease management.
- Responsible for pro-actively assessing, monitoring, providing and co -coordinating care for those patients who have Long Term Conditions/Minor Illness.
- Responsible for co-coordinating timely and effective multi-agency services to meet the individual needs to an identified population within the community setting.
- As part of monthly primary care led multidisciplinary team responsible for decisions about care and service provision for individual pathways.
- Reduce reliance on secondary care services and increase provision of care in a primary, community or home environment in order to prevent unnecessary hospital admission and conveyance.
