**Job overview**:
This is an exciting opportunity to be part of the West Suffolk Memory Assessment Service as it begins a process of transformation and development.
You will join an experienced and growing team of supportive practitioners and will contribute to the assessment, diagnosis and treatment of people referred the the service with cognitive difficulties. You will complete holistic assessments, including validated Cognitive Assessments, and participate in the formulation of diagnoses of dementia.
You will complete Triage and Duty days as part of your role, supported by the MDT as needed.
The role allows for hybrid working opportunities - patient-facing appointments in clinics or Home Visits, Duty and Triage in the office and working from home if suitable for the task,
You will receive monthly Management Supervision and will be supported to complete Clinical Supervision, individually or as part of a group.
Training and Professional development are embedded in the team's culture.
Both full-time and part-time opportunities can be considered.

- Support Student Nurses on Placement, either as Practice Assessor or Supervisor, or to support others performing these roles.
