**Job Purpose**

Reporting directly to the Housing Officer, you will provide support and management to vulnerable homeless adults living in our Supported Accommodation. You will be aware of the housing support and well-being needs of each Service User and ensure they are meeting them by putting a workable plan in place. Overtime you will develop professional relationship with external agencies that can support your Service User. With a case load of up to twenty-five people, you well be versatile and flexible in your approach. The job will also require you to be on call on a rota basis.You will report directly to the Housing Officer. Key tasks, duties and accountabilities
- Complete risk assessments and support plans for all new Service Users in your case load.
- Advise and assist Service Users with enquiries about rent payments and service charges.
- Plan and complete all units in the Independent Living Model with your Service User. Promote, supportand if necessary, organise communal activities with Service Users, to include health and well-being andeducation events, encouraging maximum use of the shared facilities by the Service Users.
- Identify, monitor and review the care and support needs of Service Users and make referrals to theappropriate statutory or voluntary agencies on behalf of the Service User.
- Liaise with the Service Users, families, carers and personnel from Health and Community Services,Community Mental Health teams, GPs and other agencies.
- Diffuse conflicts between Service Users and eliminate anti-social behaviour by ensuring procedures arefollowed swiftly.
- To recognise safeguarding concerns and where appropriate, report to the line manager to followsafeguarding procedure. Attend safeguarding meetings when requested, monitor and review wherenecessary.
- Ensure vacant rooms are suitable for new Service Users.
- Ensure Service Users are onboarded following correct procedures.
- Establish and maintain links with the local community, be aware of the resources available to ServiceUsers within the community.
- Supervise arrangements and collect monies where necessary. Maintain accurate records of all incomecollected and spent.
- Follow the accident and incident policy and procedure in case of any incidents escalating as required
- To be responsible, as far as is practicable, for the supervision, cleanliness, safety and security of thebuilding and shared areas and report any defects in the premises or equipment or accidents as appropriate. Follow up to ensure satisfactory and timely completion.
- Work in accordance with Lone Worker and Staff Protection procedures.
- To be aware of the individual well-being of each Service User by making regular face to face or verbal contact in accordance with agreed procedures. Visit Service Users identified as being ‘at risk’ daily.
- To work within the procedures, processes and guidance included in the Staff Handbook.
- To provide information, guidance and advice as appropriate.
- Prepare and review housing support plans and risk assessments, carry out any follow up actions andmonitor outcomes for each Service User.
- Work within the guidelines included in the professional boundaries procedure and ensure that thehighest standards of confidentiality relating to individual Service Users are maintained.
- Maintain an efficient manual and computerised administration system for supported housing schemes.Maintain the Supported Housing Hostel diary and record of Service User contact daily, and HostelService User records in accordance with the agreed procedures.
- Contribute to development work and projects that meet the needs of supported housing Service Users,adopt specialisms on behalf of the team and contribute to the review of procedures and processes.
- Attend staff meetings, training sessions and other meetings as required.
- Perform any other reasonable duties compatible with the nature of the post as determined byManagers within the service area.
- Undertake any additional responsibilities as directed by the line manager commensurate with the leveland grading for the role.
- Adhere to the Company’s policies, rules and procedures including, Health and Safety, EqualOpportunities, all other legislative responsibilities such as; governance, financial and procedural rules.
- Ensure compliance to the organisation’s Code of Conduct in the delivery and provision of services tostaff and the community.

**Essential**
Full driving license and own car

Salary depending on experience + mileage allowance

Licence/Certification:

- NVQ Level 2 Health & Social Care (preferred)
