ISCAN Quality Support Care Coordinator
- Band 4
Fixed Term Contract

37.5 hours per week

The post-holder will have a pivotal contribution to make in supporting the quality of every part of the ISCAN team. The Quality Support Officer is responsible for a wide range of quality support and administrative functions including data management and tracking of all Educational Health care plans (EHCP’s). The collection, collation, input and quality check of data into health systems is a critical element.

Having the oversight of the teams data, working with the supervisor to ensure it is up to date and reports are completed for the SEND strategic forums is a core requirement of the role. Accuracy and timeliness of data inputting, is therefore essential as well as competence with business systems and software packages.

Other elements of the role include support to create and manage resources, liaison with Children, Young People and their Families, and a wide range of external and internal stakeholders.

To carry out this role effectively, the role holder will expect to be based predominately with members of the ISCAN team but will need to work alongside the Community paediatric team in order to facilitate effective co-working.

This is a demanding role requiring high levels of administrative skills and communication skills and the ability to work proactively as part of a team,
initiating systems and identifying system improvements.

The role also requires some degree of patient and stakeholder
facing, requiring high levels of inter-personal skills and personal
confidence.

Having a respectful and empathetic attitude will be essential, with the capacity to manage highly sensitive, highly complex and confidential information.
The role requires the post-holder to often work alone in the office so will be frequently taking calls from different stakeholders. This requires a capacity to communicate, manage sensitive, confidential information safely and know when and how to liaise with clinical colleagues and
thereafter.

To provide a quality, comprehensive high level administrative and clerical support to the ISCAN/Community paediatric team.

To develop a good understanding of the importance of clinical and performance review requirements of the ISCAN/SEND Trust information systems.

To set up and maintain efficient office support systems, ensuring that ISCAN/SEND materials and resources are reproduced in a timely manner and efficiently organised to enable maximum clinical contact time.

To input and quality check data in the Trust information systems.

To prepare and summarise data in preparation for standardised reports.

To support, the development, of publicity and other materials for parents and stakeholders.

To provide the team with Expertise and support in the trust information systems

To manage, telephone and personal contact, from patients and carers, other staff members and external persons with intelligence, foresight and sensitivity, strongly respecting the needs for confidentiality.

To be responsible for sourcing, ordering and managing a range of clinical support and office support resources.

To support, the adherence, of local, administrative and human resource procedures to enable the team to function effectively.

To support any ad hoc administration tasks required by all teams within the Children’s and Young people’s team

Communications

Communicates, highly sensitive and confidential information with a wide range of multi-professional, inter-agency personnel including residents and members of the public

Arrange appointments for patients, using EMIS, Clinical diary management and set up reminders

Monitor Cancellation, DNA and rearranged appointment data

Implement improvement pathways for clinic bookings

Sort distribute and where appropriate deal with correspondence and messages, both verbal and written to a high quality standard.
