The post holder will be based at the North Wales Adolescent Service in Abergele and will be expected to undertake assessments and interventions under the direction of the senior nursing team.

Applications are invited for the full time Band 4 Assistant Practitioner post working with the multi-disciplinary Tier 4 CAMHS Inpatient team. The post holder will be based at the North Wales Adolescent Service in Abergele and will be expected to undertake assessments and interventions under the direction of the senior nursing team. The post holder will be required to escort patients on trips out so must have a driving licence.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

- To undertake assessment of clients through direct and indirect observations.
- To ensure appropriate clinical documentation is completed to support safe delivery of care.
- To carry out assessments and interventions as directed by the supervisor
- To be a member of the appropriate multi-disciplinary team providing services to the young people, under the guidance of the Senior Team members.
- To communicate sensitive condition related information to young people, their relatives and other professionals as directed by the supervisor. The context for this may be the client’s own home, community or Ward.
- To implement programmes of care or intervention as directed by the supervisor.
- The post holder will have a personal duty of care for Health Board equipment and facilities
- To be responsible for the planning and prioritisation of own workload and activities following the direction and guidelines given by thesupervisor.
- To actively contribute to the development and delivery of therapeutic groups to young people on the Ward under supervision.
- To be capable of working in clinical situations which require concentration and which may involve distressing or emotionally demanding circumstances or where there is risk of verbal or physical aggression
- To maintain and update client records in accordance with Health Board and professional guidelines
- The post holder may be required to demonstrate their own duties to others
- To receive regular clinical supervision in accordance with professional practice guidelines
