The Patient Navigator role post holder will be responsible for supporting patients and their will be responsible for supporting patients and their support network through the Rapid Diagnostic Service for suspected lower GI cancer across secondary, community and primary care.

The Navigator post holder will support patients and their support network from initial referral onward to first definitive treatment.

The Navigator will work directly and indirectly with patients and their support network to assist in coordinating their care through their diagnostic journey. Identifying, escalating and managing issues that patients may have with transport attending appointments, lack of understanding about diagnostic investigations and any additional support requirements.

The navigator post holder will be involved from the beginning of the cancer pathway, GP referral into the cancer pathway at Blackpool Teaching Hospitals.

An integral part of the role will be supporting multi-disciplinary team (MDT) and wider clinical team in implementing the rapid diagnostic service (RDC) for patients with suspected cancer to achieve a definitive diagnostic by day 28.
- Provide empathy and support to patients and their carers to establish positive relationships
- Key point of contact for patients, patient support network and health care professional during diagnostic and treatmen
- Effectively communicate with patients to ensure they are well prepared for their clinical investigations (e.g., Endoscopy), providing appropriate advice and information regarding all aspects of investigations and appointments
- Liaise with patients and / or the clinical team to effectively communicate any follow up appointments, scans and admissions to ensure a smooth transition along the pathway from diagnostic to decision to treat and beyond into treatment activitie
- Assist patients and their support network to access appropriate informatio
- Support patients and their support network by sign posting to a range of support services (e.g., psychological support, post diagnostic support or lifestyle changes

The post holder will act as the administrator supporting the clinician’s managing patients journey through the pathway.

The post holder will be required to use the Somerset Cancer Database to record data.

It will be part of the Navigators role to help improve both patient experience and patient compliance, by coordinating care to reduce the number of appointments that are missed by patients (both outpatient and diagnostic investigation).

The post holder will work as a member of the cancer MDT and wider clinical team, working closely with MDT Coordinators, Cancer Trackers, Clinical Offices, Appointments Centre, Central Booking Office, Diagnostic and Therapy Departments to ensure patients are actively managed through the prostate cancer pathway.

The post holder will be expected to work across a range of IT systems to coordinate booking of patient appointments.
