Assistant Practitioner Collaborative Care Team

**Job summary**
The collaborative care team covers North Oxfordshire and South Northants, working remotely to see vulnerable patients in their own homes. The team consists of nurse lead, occupational therapist, assistant practitioner and a support worker. The team meet regularly to review patients and allocate caseloads to the most appropriate practitioner. The assistant practitioner will work alongside the established team to support the development of the service.
**Main duties of the job**
**The post-holder will**:
Assess, signpost, support and monitor patients and their carers with the aim of enabling them to self-manage their long-term condition, enhance quality of life and prevent hospital admissions.
Assist and support registered practitioners in all aspects of service delivery.
Work within own competency and without direct supervision, undertaking duties including clinical and therapy led interventions delegated by the registered clinicians.

PML holds GMS contracts, as well as being a GP Federation representing c. 50 GP practices covering approximately 650,000 patients.

**Professional / Ethical Practice**
**This involves**:
- Acting in accordance with Principal Medical Limited (PML) protocols and policies and the Law.
- Obtaining informed consent from the patient before initiating any procedure or intervention and documenting this within the patients notes.
- Maintaining confidentiality and communicating patient information in accordance with the Caldicott (1997) principles.
- Working in a non-judgmental anti-discriminatory way, with regard to the cultural and religious beliefs of individuals and groups.
- Working within boundaries of competency and knowledge at all times.
- Escalate concerns in line with PML and national guidance.
**Patient Care**
**This involves**:
- Appropriately assessing patients and their carers within sphere of competence, including physical and mental health assessment alongside social care needs.
- Making appropriate referrals to relevant partner agencies with the patients consent.
- Reporting back information from patient and professional contact to ensure consistency of patient care.
- Seeking advice from a registered practitioner for matters outside the post holders sphere of responsibility, knowledge, or skills.
- Understanding where to go for support when needed and as appropriate.
- Educating, supporting, and advising patients and their carers appropriately and within own competency and knowledge.
- Maximising capacity through regular communication with service registered practitioners.
- Maintaining accurate patient records within agreed timescales and utilising relevant IT systems.
- Establishing and carrying out routine monitoring of patients
- Participating in clinical audit as deemed appropriate.
**Communication**
**This involves**:
- Liaising with other related professional and voluntary disciplines.
- Actively listening and responding to what people are saying and evaluating their needs.
- Checking information from other people and confirming its accuracy.
- Coordinate, organise and prioritise own workload on a daily basis.
- Support new staff to ensure compliance with induction and signpost to support as necessary.
- Contribute to new ways of working, offering ideas and support for change.
- Act as a role model at all times.
- Reporting any issues in the workplace that may put health and safety at risk.
- Contemporaneously updating GP records in relation to CCT activity.
**Care / Caseload Management**
**This involves**:
- Prioritising own workload to ensure that support to patients is consistent and appropriate.
- Attend team MDT meetings to ensure coordination of patient caseloads.
- Attend and represent service at regular GP MDT meetings, assisting with patient allocation.
- Having personal responsibility for ensuring that all data relating to activities, including contact with other disciplines, is recorded accurately and promptly so that the information system is kept up to date.
- Undertaking work activities and related training as defined in health and safety policies and procedures.
- Assisting in maintaining a safe working environment for self, colleagues, patients, and others.
- Being aware of and abiding by Lone Worker policy.
- Summoning immediate help for any emergency and taking appropriate action to contain it.
**Personal / Professional Development**
**This involves**:
- Attending staff meetings, team meetings and in-service training as directed
- Participating in annual appraisal to identify personal / professional development
