The UK's largest provider of health and wellbeing services are looking for a Senior Outreach Wellbeing Practitioner to build a team that will improve the health and wellbeing of hard to engage community groups.

As part of the team servicing Essex, the role will be home based but you will need to have the ability to travel regularly throughout Essex, working in the community.

As the Senior Outreach Welling Practitioner, you will take a lead role in designing and delivering a range of health and welling promotion interventions across Essex. You will lead a small team of Outreach Wellbeing Coaches to develop and deliver a tailored 1:1 coaching programme which aims to support clients' self management, confidence and accessing health checks.

The team will work closely with target populations to gain trust and engagement and to find people who will benefit support but are not likely to access the service via the normal referral routes. The focus groups in this role will the following:
Homeless, Criminal justice system, Traveller communities, Racially Minoritised Groups, Drugs and Alcohol misusers, LGBTQ+, People who have lost trust in services.

In this role, you will also establish and maintain professional relationships with partner agencies to facilitate outreach interventions, including developing and delivering training sessions for practitioners and service users when needed.

To be considered, you'll have in designing, delivering, and evaluating targeted health promotion community services, working with a range of partner agencies and stakeholders. You will be someone with experience of managing or supervising a small team and importantly, you will have experience of working in 'hard to engage' groups such as those mentioned above and will have a good level of knowledge regarding national and local health priorities, and the needs of target populations.
