The role of Care Coordinator is a key role to Cedar Oak Healthcare that will require the individual to work alongside the management team to ensure the smooth running of visits to our clients. For the Care Coordinator to perform effectively, the successful applicant will have had previous experience in a Care Co-ordinator or similar role and be able to adapt to changes and deal with pressure and will also possess the following key attributes, which must be demonstrated at the interview:

- Essential Skills and Experience
- Must have a full drivers licence
- Must have own car
- Previous experience as a care coordinator is ESSENTIAL

The Care Coordinator's duties will include:

- Accepting care packages and assigning care workers to service users in an appropriate manner
- Producing care workers' rota and managing the roster software with People planner
- Using electronic monitoring software to monitor care workers' attendance and timeliness for visits
- Arranging care reviews and risk assessments for service users with the field supervisor
- Conducting telephone monitoring and ensuring service quality
- Experience in project support and logístical planning with the ability to work independently, with mínimal supervision and to remain calm in stressful situations
- Speaking with service users and families over the phone.
- Be a part of the weekly ON CALL once per month.
