CARE COORDINATOR

**JOB PURPOSE**: To oversee/maintain the care of the Domiciliary Service’s Clients and take responsibility for the day-to-day running of the Domiciliary Service.

**CORE RESPONSIBILITIES**
- Ensure all staff members contribute to the best of their ability to the efficient running of the Domiciliary Service.
- Cover client care calls as per weekly/daily rota on Care Planner.
- Conduct spot checks, supervisions, client reviews & feedback, medication, moving & handling competency checks.
- Facilitate the practical induction of all new care & support workers.
- Ensure to carry out monthly support calls to assigned service users and update the system.
- Assess social and health care needs of new clients and maintain ongoing assessment and review of all other clients (in the absence of, or as requested by, the Registered Manager).
- Liaise with senior staff members and care staff to ensure that the highest standard of service is provided to service users.
- Allocate suitable alternative care & support workers when an existing care & support worker notifies that they are unable to attend a call.
- Undertake direct domiciliary care as required. Assist clients in all aspects of their care needs.
- Practice maximum integrity in all dealings with clients' personal and financial affairs and avoid abuse of the privileged relationship that exists with clients.
- Undertake initial risk assessments within clients’ homes in line with the Company’s Health & Safety policies and procedures as requested by the Registered Manager.

**HOURS**:
Full-time, 37.5 hours per week.

**REQUIREMENTS**:

- Minimum of 1-2 years of experience delivering care and understand how to coordinate and motivate a team.
- Extensive knowledge of Care Planner (1 year min. required).
- Be motivated in developing teams and will lead from the front.
- Have experience in managing carers

**Salary**: £24,000.00-£26,000.00 per year
