**Support/Case Worker
- **We are seeking a compassionate, self-motivated, and proactive individual, who has excellent interpersonal and communication skills and gains satisfaction from helping others.

**Location**: Leeds -Hybrid (home and office working)

**Hours**: 37 Full time

**Contract**:12 month contract initially, with potential for the role to be permanent

**Salary**: £22875 to £26000 dependant on experience

**About the role**:
Care and Repair provides the Homeplus service throughout Leeds. This service promotes independent living through facilitating return home from hospital, improving health and security at home and helping to prevent falls and cold related health conditions. Our partners throughout the city refer clients, some of whom are recently discharged from hospital to the Homeplus service. We then provide a variety of services that support their recovery and enable them to thrive in their own home. Our services would not be possible without our fabulous team of Support/Caseworkers who are 100% committed to helping some of the most vulnerable people in the city of Leeds.

You will be supporting people with a wide range of issues, communicating sensitively and effectively to identify their needs. You will support a safe return from hospital by assessing clients in their own homes, identifying and delivering the interventions needed to keep them safe and warm and able to remain independent in their own homes for longer.

You will be IT proficient, including the use of Microsoft Office programmes, and be physically able to lift, carry and fit items of falls equipment such as raised toilet seats and bath boards. Full training will be given including trusted assessor training and in-house training on adaptations and assessment within the home.

**Key responsibilities include**:

- Manage a case load of visits, assessments, and follow-up actions for both Enhance and Homeplus services
- Assess minor disability equipment needs, deliver and fit equipment in clients' homes
- Maintain detailed and accurate case notes on the Customer Relations Management system
- Work alongside our Customer Service Team, complete client feedback calls on a weekly basis to gather information on the client experience and creating case studies to share with funders

**About you**:
**Other essential skills and experience include**:

- Ability to communicate effectively with people from various backgrounds and cultures,
- Highly organized, self-motivated, and able to use own initiative to manage a wide variety of tasks efficiently and effectively
- Possession of a full driving license and have access to a vehicle.
- IT literate, able to use Microsoft Word, Excel amongst and able to use other digital methodologies to record information and data
- Understanding of handling sensitive and confidential information
