1. Objective of the post

You will join a team of committed professionals in our client's Community Support Services Team in an exciting role that will provide information, advice and practical and emotional support to older adults,who are in the next stage of the care journey.

We recognise the challenges faced by clients and carers when trying to negotiate their long-term care placement, either during a planned move or in crisis for example after a stay in hospital.

The role will directly help and support clients and families to make an informed choice about care, funding and how to choose the right care home. Once at their new home this role will continue to support the client for as long as required, providing befriending, transitional help and in-reach culturally appropriate support.

2. Prime duties of the post
- To promote and support LJWB’s Vision: Positively Changing Lives.
- To work with statutory professional, clients and their families to proactively source a range ofcare provision following either significant changes in a person’s care needs or significant life events such as discharge from hospital or loss of a family carer.
- To work as part of a team enabling the achievement of positive outcomes through a person
- centred case management approach, ensuring the needs of clients are met effectively and in a timely manner through delivery of a range of high-quality interventions and case support.
- To respond to referrals, assessment of needs and work in partnership with individuals, their families, and carers, to build up care options and once these have been agreed to support the client during the care transition and thereafter as required. This may for example be continuing to visit and support the client after their have moved into a care home, ensuring that they receive culturally appropriate care and support.
- To support and ensure compliance with the delivery of LJWB’s Older Person’s Strategy, improving well-being, reducing inequalities, and delivering a wide range of accessible, creative and innovative services that add value to, strengthen and support clients and the community to keep well, remain independent and maximise potential.
- To champion services and support for Older People working closely and in collaboration with colleagues, partner organisations, community groups and volunteers to develop a range of preventative community-based initiatives, interventions, strategies and programmes to improve well-being.
- To ensure the provision of effective services to meet Leeds Jewish Welfare Board’s (LJWB) charitable objectives and meet the needs of the community.
- To undertake any other reasonable duties as required which are appropriate to the work of the team and are commensurate with the responsibility and level of the post.
- To comply with Health and Safety legislation.
