This is an exciting opportunity to be involved in a brand-new project in Yorkshire. The project is funded by SQuIRe (Stroke Quality Improvement for Rehabilitation) establishing two new Stroke Recovery Services in Bradford and Doncaster.
Our Stroke Recovery Services support stroke survivors and their families from hospital to home using a person-centred approach to address the practical and emotional needs of stroke.
We’re looking for innovative, passionate and professional individuals with excellent communication, interpersonal and organisational skills to join us in delivering this project.
Key responsibilities will include:

- Setting up and establishing new services, working with external stakeholders to achieve this.
- Working with stroke survivors and carers to identify their needs, providing person-centred support which enables them to achieve their desired outcomes.
- Delivering an effective service in line with our case management principles, ensuring that confidential and accurate records are kept on our CRM database.
- Supporting clients to make informed lifestyle changes which will help them to prevent further strokes.
- Working collaboratively with other professionals and organisations involved in the care of stroke survivors and carers to ensure the best possible support is provided throughout the stroke pathway.
- Contributing to the delivery and evaluation of the project.

The post holder will have experience/background in:

- Experience in providing person-centred support.
- Experience working with people with a disability or long-term health condition, and their carers.
- Experience in using technology and IT systems to support your work and keep timely, accurate records.
- Effective listening skills with the ability to communicate clearly with a wide range of people.
- Experience in working with health and social care professionals in a variety of settings.

**Our services are contracted; we currently have funding for this contract until March 2024.**
To fulfil the role, you must be a resident of the UK and have the right to work in the UK.
