The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

Reporting to the Service Delivery Coach, the Stroke Association Support Coordinator will:

- Support stroke survivors and their carers from hospital discharge into the community.
- Provide personalised information, emotional support and reassurance to stroke survivors and their families.
- Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
- Deliver group-based sessions to stroke survivors and their carers.

The post holder will:

- Have a caring and empathic approach to supporting people.
- Have experience in supporting people in challenging situations.
- Be experienced in empowering people with emotional support needs.
- Be resilient and able to manage their own well-being.
- Have an organised, flexible and adaptive approach to their work.

**Our services are contracted; we currently have funding for this contract until 31 July 2024.**

To fulfil the role, you must be a resident of the UK and have the right to work in the UK.
