The project is funded by SQuIRe (Stroke Quality Improvement for Rehabilitation).

This is an exciting opportunity to work with stroke survivors, their families and carers to help them rebuild their lives following a stroke.

As a Stroke Association Support Coordinator, you will be:

- Supporting new stroke survivors and their carers from hospital discharge into the community.
- Providing personalised information, advice and support.
- Support clients to make informed lifestyle changes which will help them to prevent further strokes.
- Delivering group education sessions to stroke survivors and carers across Cumbria.
- Working with other health and social care professionals across the stroke pathway to ensure high-quality support.
- Be accountable for volunteers, so experience in recruiting, training and managing volunteers would be an advantage.

This role will suit a person who has:

- Experience/a background in a caring profession, ideally supporting people with disabilities.
- Excellent IT skills and an ability to maintain accurate records.
- An affinity with the values of the Stroke Association.
- A flexible approach and an ability to effectively manage a caseload.
- Experience in setting up and running support groups.

**This is a fixed-term position until 31 March 2024.**

**Our services are contracted; we currently have funding for this contract until 31 March 2024.**

To fulfil the role, you must be a resident of the UK and have the right to work in the UK.
