The Health Care Assistant will be responsible for providing skilled nursing care to the residents of Tower Hamlets, promoting the health of the population and reducing the effects of ill health and disability and maximising the individuals potential for independence. S/he will work closely with General Practices, Social Services and other statutory and non-statutory services to meet the needs of the local population using public health information to support service development. You will be part of the team who ensures all new admissions to the Community Nursing services have a clinically robust assessment and patient focused care plan.

To work as a member of the Community Health Team, providing rehabilitation, nursing care and to patients in their homes, community care settings and/or in a group setting.

The Community Health Team (CHT) will manage via a single locality/Network caseload all patients along a step up/down model which will incorporate care from a two hour response, short interventions, rehabilitation, through to the management of Long term conditions and end of life care.

The aim of the team is to maximise the independence, health, and well-being of adults in order to enable them to return to independent or supported living in the community wherever possible.

The CHT works with adults in the community who have various needs such as long term neurological conditions, recent surgery, risk of falls, palliative care, wound care needs, reduced physical and functional ability. CHT is a 7 day service operating 8am -8pm.

Join our dynamic healthcare team, bolstered by a comprehensive primary care unit featuring a skilled ensemble, including a physiotherapist, an occupational therapist, care navigators, and administrative professionals. Our nursing component places a strong emphasis on collaboration with senior nurses and families to empower individuals towards independent living.

Our nursing team boasts exceptional talent, with 1 Band 8A, 2 Band 7, and 5 Band 6 nurses, forming the core of our senior nursing team. Complementing them are 6 band 5 nurses, 2 Band 4 SHCA, and 4 HCAs with nursing skills, providing a robust framework for peer support and continuous development. The integration of social services, voluntary sector, and health expertise ensures a holistic approach in crafting patient-focused care plans.
