**Job description Job responsibilities JOB PURPOSE**: Phlebotomy sessions and clinical work to support the practice to provide defined and delegated aspects of patient care (Chronic disease and Health reviews and Injections) under the supervision of a qualified nurse and GP Lead at the practice and support the GP practice clinical team by undertaking a range of clinical support functions. SCOPE: Undertake clearly defined and controlled clinical tasks for named patients referred by a health care professional. You will work in accordance with specified local and national protocols and guidelines. MAIN DUTIES AND RESPONSIBILITIES: Undertake specified clinical activities and tests for named patients, within the scope of established clinical protocols, policies and procedures, including venepuncture, urine testing, pulse, blood pressure readings, weight & height and BMI monitoring, memory assessments and basic HCA medicals.

Undertake a range of clinical support functions including preparing and stocking your clinical area in line with individual practice protocols. Provide relevant health information to patients and carers within defined protocols. At all times observe infection control procedures to protect yourself, patients and colleagues. Assist in maintaining a safe working environment for yourself, patients, carers and the multi-disciplinary team, alerting other members of the Practice team to issues of quality and risk identified in work with patients.

Work within established systems of clinical governance, contributing as appropriate to the practice including audit activity. Managing clinical waste as per practice guidelines. Convey relevant information and receive feedback from Nurse / Clinical Pharmacists / GPs at the practice. Order and maintain stocks of equipment as directed / required.

Have a full awareness and understanding of the current GMS contract as well as DES / LES and QOF requirements for the practice. ORGANISATIONAL Practices and promotes confidentiality at all times Maintain accurate records of patient contacts, providing clear written and verbal information as required by the individual practice. Refer back to other members of the clinical team any patient that doesn't fit within established protocols. Actively participate in the smooth running of the service within the practice as an NHS organisation.

**SYSTEMS AND EQUIPMENT**: Ensure that all patient information is properly recorded and read coded within the patient record using the clinical software provided within the practice. Actively participate in the care and maintenance of equipment, demonstrating economy in the use of supplies DECISIONS AND JUDGMENTS: Work at all times within the established policies, standards and guidelines of the practice as well as local and national guidelines from all NHS and clinically recognised organisations. Undertake clearly defined clinical duties that have been agreed, taught, supervised and signed off. Refer back to a health care professional any patient where an informed clinical judgment is required before proceeding with any treatment.

**COMMUNICATIONS AND RELATIONSHIPS**: Actively communicate with all colleagues, patients, relatives, carers, visitors and other agencies in a professional manner at all times. Communicate effectively with patients and carers, recognising patients' rights and responsibilities. Work as a member of the practice nursing team, seeking advice, guidance and support from other members of the team within the practice. Communicate concerns identified in patient contacts to other health care professionals in the team.

Work under the supervision of a named health care professional, who will ensure that skills are maintained and developed to meet the needs of the post. PERSONAL DEVELOPMENT Actively participate in the In-Service training opportunities available. Maintain responsibility for own personal development under supervision and guidance of the Lead GP Principal. Demonstrate a positive attitude to suggested changes and development in practice through the introduction of evidence-based good practice.

Required to undertake all annual Mandatory training, e.g. Basic Life Support, Moving & Handling, Fire, Hygiene, NHS Health Checks etc. MOST CHALLENGING / DIFFICULT PART OF THE JOB: The demands of this post require a person who works well under pressure and who is enthusiastic, self-motivated and who can remain calm in sometimes stressful situations. Due to the nature of the post, the post-holder will be exposed to blood and other bodily fluids and therefore will be meticulous in adhering to universal hygiene precautions.
