Clinical Practice or Work Practice To visit service users in their homes.To provide assessments, interventions and reviews in a person centred and flexible way.To ensure robust risk assessment are completed.To recognise limitations and refer to other team members as appropriate.To work independently and manage own work/caseload.To respond appropriately to urgent and non-routine situations.To ensure accurate record keeping and completion of electronic recording requirements.To ensure confidentially is maintained at all times.To contribute to reviews and accurately report back in a timely manner.To provide support and training to other team members as required.To participate in non-clinical team meetings in order to contribute to the development and effectiveness of the service.To work independently and as part of a team.To actively contribute to the development of the Assistant Practitioner role.To undertake any other reasonable duties as requested by the Team Leader. Health and Safety To work within all Trust guidelines in relation to infection control, manual handling, lone working and fire.To report to clinician in charge any untoward incidents or near misses. Education and Development To identify with clinical supervisor training and development needs.To participate in mandatory training and developing of nursing and therapy related competenciesAbility to recognise own limitations and seek appropriate support. Policies and Procedures The duties of the post will be undertaken in accordance with the policies, procedures and practices of the Trust.

It is the post holders responsibility to ensure they keep up to date with these policies and other documents.
