Patient Tracker

The Emergency Department at Leighton Hospital is seeking to recruit a highly committed and enthusiastic Patient Tracker to join their dynamic and supportive team. The role holder will be responsible for co-ordinating patient flow through the department and ensuring that this is done in a safe and efficient manner.
This is an exciting and fast-paced role within what can often be a very busy and stressful environment. We are looking for an organised, professional and quick-thinking individual who has the ability to manage multiple tasks in a timely manner. You will be confident in communicating with patients and colleagues of all levels and comfortable in using IT systems including PCS and an electronic live status board.
Please note that full training will be provided within the role, although previous NHS experience is considered to be advantageous
Working closely with the clinical team, the successful post holder will be required to track specialty reviews, bed requests, and moves in and out of the department, as well as to monitor performance metrics and contribute to the excellent quality of care delivered to patients by maintaining a detailed overview of all activity within the department.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.
We employ almost 5,000 members of staff, provide around 540 hospital beds, with a range of services including accident and emergency, maternity, outpatients, therapies, and children's health.
The Trust is also part of Central Cheshire Integrated Care Partnership (CCICP), a unique local health partnership that provides a range of community services for people across South Cheshire and Vale Royal.
During the Coronavirus pandemic, the Trust was recognised nationally for its 'Be Safe Be EquiPPEd' campaign, which aimed to make the Trust as safe as possible for staff and patients, through supporting the correct use of PPE. The results in the 2020 national NHS Staff Survey showed improvements in all elements of the safety culture theme, with 92% of respondents feeling their role made a difference to our patients. We were also recognised nationally for our workforce health and wellbeing initiatives.

The post holder will provide a comprehensive, quality, confidential service to the clinical and managerial teams. The post holder will be responsible for reviewing waiting times, identifying areas of delay and ensuring the timely throughput of patients within the Emergency Department. This will include ensuring escalation procedures are activated when necessary and all relevant issues relating to achieving the 4 hourly target are addressed. It is
