We are looking for an experienced mental health professional to join our friendly, and supportive Link Worker team which is part of the Primary Care Network Mental Health Teams (PCNMHT’s) covering East Suffolk. The team provides a point of access into mental services for people experiencing a wide range of mild-moderate mental health problems in primary care. Link Workers are usually based in GP surgeries across the county and offer a face-to-face/telephone assessments and brief interventions for patients in primary care. Our base sites are located in Ipswich. We currently have Link Worker vacancies at two locations in East Ipswich PCN and East Suffolk PCN.

Applicants are expected to hold a core professional qualification (Mental Health Nurse/Social Worker/Occupational Therapist) and should have at least one year’s community experience.

As a Mental Health Link worker, you will be working collaboratively with people aged 16 and above, presenting with a range of mental health needs within a primary care environment. The post holder requires to carry out mental health assessments, identify appropriate treatment pathways depending on the persons needs, offer brief interventions and/or signpost to the most suitable service provider. Mental Health Link Worker’s work alongside GP's, and other primary care staff, and liaise with colleagues from multiple sectors, both statutory and non-statutory, to provide expert advice and education in accordance with evidence based NICE guidance.

NSFT is committed to lifelong learning and continuous professional development (CPD) for its entire staff. There are a variety of definitions of CPD across the professions, but it is usually taken to mean learning activities which update existing skills.

Clinical
- To provide specialist mental health advice and undertake assessments with frontline workers, where the mental health needs of the individual referred cannot be resolved by primary care non specialist workers..
- With staff from other agencies, carry out mental health assessments of people, which will include a systemic formulation and advice on evidence based therapeutic interventions
- To provide referrers and other professionals with written summaries of assessment, formulation and treatment outcomes
- To participate in the locality model by undertaking direct work where clinically indicated using evidence based therapeutic interventions to improve mental wellbeing
- Provide professional telephone advice for those working in Suffolk who require specialist advice and information on a wide range of mental health/wellbeing issues
- Support the Trust response to external referrals. by working in partnership with other NSFT clinical teams.
- To maintain clinical and professional responsibility for a defined workload.
- To liaise with colleagues and other professionals or agencies as required, including attending case conferences or planning meetings as part of the Multi agency working model
- To promote clinical effectiveness by the use of clinical protocols and clinical audit.
- To refer service users to alternative care where detailed assessment indicates a different level of service.
