An exciting opportunity has arisen within the Central West Community Mental health team for a Physical Health support worker

The post holder will work alongside the Community Mental Health Team under the supervision of the team manager and the physical health lead.

The post holder will work alongside the Community Mental Health Team covering Central West Community Mental health team, under the supervision of the physical health lead and team manager, undertaking physical health checks, working within physical health clinics and completing general health and wellbeing assessments.

The post holder will assess physiological observations and promote healthy lifestyles for our service users. The post holder will assist in enabling service users to access the support they need to recover from their mental health and physical health problems, promoting recovery in order to return to everyday living. They will also assist care co-coordinators in working across boundaries of care to plan, implement and evaluate care within care pathways as part of the stepped care model.
