If you are a Rehabilitation Assistant who is interested in client centred care within the community, we are looking for an enthusiastic and motivated individual to join the Central Lancashire Rehabilitation team (part of the Lancashire Care Foundation Trust - Community Wellbeing Network).

The team is an integrated multi-disciplinary team of Occupational Therapists, Physiotherapists, Nurses, Assistant Practitioners and Support Workers.

The service operates 8.00am - 8.00pm 365 days a year.

You will need excellent communication skills and have experience of working as part of a team.
