Cornwall Health for Homeless is pleased to offer a rare and unique opportunity to work flexibly, creatively and rewardingly with some of the most disadvantaged individuals in our county.

At present we are looking for a Trainee Advanced Clinical Practitioner to fill a vacancy in our small team of GPs. This is a developmental role moving to a Band 8a Advanced Clinical Practitioner upon completion of training.

We are an EMIS based practice, with approximately 250 registered patients distributed across Cornwall. All our patients are either street homeless or unstably housed. We currently offer both drop-in and booked appointments at each of our three clinic sites in Truro, Pool and Penzance. As might be expected, home visits are rarely needed.

Health for Homeless is very unusual in being block-funded and therefore not required to claim for capitation, QOF or other payments. We have our own small, admin team supported by a part-time office manager. In addition, we have more extensive backing from Cornwall Partnership Foundation Trust, which includes the assistance of a specific mental health team dedicated to the needs of the homeless population.

It is a full time substantive, salaried position with the potential to adapt regular hours to suit the individual applicant. There is no requirement to work in the evenings, at weekends or on Bank Holidays.

**Main duties of the job**:
The post holder will be responsible for working clinically autonomously as an Advanced Clinical practitioner within primary care to provide patient centred care to undifferentiated and undiagnosed presentations.

Patient need is high, both in terms of physical and psychological morbidity. Skill is also necessary in the management of long-term addictions (for which training is available). Successful practitioners require excellent interpersonal skills to establish good patient rapport, as well as an interest in pursuing a tolerant, empathic and holistic approach. Multidisciplinary working with a broad range of other organisations is essential.
