Sirona Medical's Social Work Department are recruiting for a Social Worker for the Hospital Team to support and guide any Bracknell residents out of Frimley Park Hospital A&E and AMU. The posts will be based at Frimley Park Hospital Monday to Friday, coveringcore business hours. Due to continued pressures on the hospitals, people are being discharged home once medically optimised. There is extreme pressure on acute beds and we want to work with the trust in supporting a timely and smooth discharge. There is arequirement to be based within the Discharge team at Frimley Park Hospital but you will be line managed by the Hospital team based in the Adult Community Team in Bracknell.
- A relevant Social Work qualification (BA/MA Social Work/DipSW or equivalent)
- Be Social Work England Registered
- Be eligible to work within the UK
- Experience of working in Hospital Discharge
