Senior Health Care Assistant

**About the role**
We are looking to recruit an experienced, passionate and reliable Senior Health Care Assistant to be part of the nursing team delivering care to patients in their own homes.
You will be responsible for supporting the Registered Nurse, who remains accountable for patient care, in the planning and delivery of patient care as detailed in individual care plans by carrying out specific tasks as delegated and reporting significant findings within span of duty.
This is an excellent opportunity for a motivated and dedicated individual to join our well established team to support and assist in the provision of nursing care for patients in their own home.
