**Job Summary**
**Full time position with on call responsibilities** (on call means that you are available by phone or in person just in case someone does not show up for work or calls in sick).
**Salary Annually: 24,000 - 27,000**

**The successful applicant will lead**:
- on assessments in the community (client reviews)
- compile care packages,
- participate in the on-call rota,
- carry out risk assessments,
- carer supervisions,
- spot checks,
- support planning and dealing with local authorities workers and other healthcare professionals,
- quality control,
- include report writing and attending Social Services meetings,
- informing/Communicating with NOK/service users any changes affecting their care Health and Safety,
**Other duties will include but not limited to**: rotas and booking carer holidays, planning, allocating and evaluating the workload of the care/support workers to ensure a safe and effective service delivery. Attention to providing continuity of care/support to individual service users. Ensuring that any sickness/absence is adequately covered. Participate in the recruitment of care workers in line with company policies and guidance and ensuring that all recruitment checks are satisfactorily completed.
