Job Overview
Randstad Agency is seeking an experienced and dedicated Adult Social Worker to join our team for short-term hospital assessments in the Eden area.
This role involves conducting assessments for adults in hospital settings, ensuring timely and effective discharge planning, and coordinating care post-discharge. The ideal candidate will have a strong background in social work within healthcare settings and a commitment to improving patient outcomes.

Key Responsibilities
Hospital Assessments: Conduct comprehensive assessments of adults in hospital settings, evaluating their needs, risks, and strengths.
Discharge Planning: Develop and implement discharge plans in collaboration with healthcare professionals, ensuring safe and timely transitions from hospital to home or other care settings.
Coordination of Care: Work with multidisciplinary teams, including doctors, nurses, and occupational therapists, to coordinate care and support services for patients.
Crisis Intervention: Provide immediate support and intervention for patients experiencing crises, ensuring their safety and well-being.
Advocacy: Advocate for patients' rights and needs, ensuring their voices are heard in care planning and decision-making processes.
Documentation: Maintain accurate and detailed records of assessments, care plans, and interventions, ensuring compliance with relevant legislation and policies.
Qualifications and Skills

Education: A degree in Social Work and current registration with Social Work England.
Experience: Significant experience working as a social worker in healthcare or hospital settings.
Ability to work effectively as part of a multidisciplinary team.
Knowledge of relevant legislation and best practices in adult social care.
Commitment to safeguarding and promoting the welfare of vulnerable adults.
