NHS Lincolnshire CCG has developed an Elective Activity Coordination Hub (EACH) to support patients in accessing the most appropriate appointment for their care. The role of Patient Referral Service Facilitator involves working in a team using an electronic system (eReferral Service) to book patient's appointments for further tests/treatment once they have seen their GP. You will be expected to liaise with patients by telephone to ensure suitability of the appointment and to give them any necessary information they require to make an informed choice (current waiting times for example).
