We currently have an exciting opportunity for a dynamic, highly motivated, innovative and forward-thinking individual to join our Community Nursing Teams, based in Carshalton or Sutton as a Band 8a, Primary Care Network Multi -Disciplinary Team/ Virtual Ward Clinician.

The definition of the virtual ward is that this model provides supportive hands-on care and assessment for up to 6 weeks with the MDT clinician working closely with the patient’s own GP and other specialists as appropriate to ensure that the patient recovers from their episode of ill health. Eligibility criteria for acceptance onto the virtual ward is that the patient is over 18 years of age, has a GP registered in Sutton and does not need intensive three or four times a day care, but needs daily care and assessment. Patient cohorts include those with long term conditions, frailty, covid/ post covid and other disease comorbidities as agreed with the patients GP.

You will be based in one of our friendly Community teams who provide a 7 day service to provide holistic assessment and care in the patient’s own home and residential homes within the borough. You will work with the Community Nursing Team, Band 6 urgent care response nurses, specialist nurses, GP's and intermediate care services to develop and support clinical management plans to enable safe care within patients own homes. You will also work closely with partners in social care, as well as other planned and unplanned services within community such as Dietetics, Podiatry, Falls Team and the Continence service. Clinically you will be working with patients who require acute nursing support at home, particularly those with complex needs and long-term conditions and multiple comorbidities.

Epsom and St Helier University Hospitals NHS Trust provides a range of medical services to approximately 490,000 people living across southwest London and northeast Surrey. In addition, we provide more specialist services, in particular renal and neonatal intensive care, to a wider area, covering parts of Sussex and Hampshire.

The Trust employs approximately 5,000 staff across its hospitals and is supported by over 500 volunteers.

The post holder will be expected to lead the clinical element of the virtual ward for the respective PCN locality, working with the patient's GP to ensure that the patients are assessed, managed and monitored and patients are also supported to find solutions to complexity, whether health, social or environments.

**Main responsibilities**: triage patients in a timely manner, provide expect care planning of complex long term conditions and comorbities, use advanced clinical level skills to assess, diagnose, prescribing treatments as necessary, line manage Band 6 urgent care response nurses.
