**Location**: Office base: 26 Harpur Street, Bedford. Flexible location working in the Community, within Service User homes & other locations as appropriate.

**Salary**:£24,179 per annum

**Working Hours**:37 hours per week, including at least 2 late evenings

**Main Purpose of the Job**

As a Floating Support Caseworker for SMART, you will have the opportunity to make a significant difference to those who are entering into accommodation, helping someone to set up a home and sustain their accommodation. You will provide trauma-informed, person centred support to individuals, developing support plans based on their individual needs & circumstances. You will work closely with other partner agencies, liaising and referring as needed. Your knowledge of financial support available and creative thinking will enable people you work with to start to build a home. You will work with people to build a positive environment for themselves, working towards life goals that support them to sustain their home and circumstances long term.

**Role responsibilities**:

- Support individuals accessing emergency accommodation to move on as quickly as possible to suitable longer-term accommodation to suit their needs and wishes
- Support to identify and move into longer-term accommodation, in partnership with other professionals to ensure a smooth transition
- Provide on-going support for people who have moved into independent accommodation (I.e private rented) to continue working towards goals
- Tenancy sustainment skills to prevent further homelessness i.e. coordinating and setting up rent payments, utilities, budgeting, reporting repairs, and organising Universal Credit payments
- Enable access to specialist support such as education, leisure, culturally specific services, employment opportunities, etc
- Support individuals to maximise and maintain income - including welfare & benefits, identifying opportunities & access to ETE courses and services
- Providing crisis intervention support if an individual is at risk of losing accommodation, e.g. debt/arrears notices, potential eviction, loss of amenities, etc.
- Carry out detailed assessments identifying priorities and ongoing need
- Develop person-centred care plans, helping people to identify their goals and move toward achieving them
- Carry out assessments identifying risks, creating an initial risk management plan to mitigate risks as much as possible.
- Ensure safeguarding concerns are raised at the earliest opportunity to adults and children’s safeguarding teams where appropriate.
- Ensure timely and accurate case notes, support plans and documents are recorded for all individuals.
- Support and encourage individuals to access specialist support, where desired, e.g. Mental Health support, drug and alcohol support, harm reduction advice and needle exchange (where required).
- Support service users to gain documentation (e.g. passports etc) where required

- To keep abreast of new developments, research and updates to policy in the field of housing, homelessness and complex needs
- Support the wider SMART RSI Team where required to ensure full service operation
