We are currently looking to recruit an enthusiastic and motivated Health Outreach and Inclusion Worker for the Health Outreach and Inclusion Service (Monday to Friday between 0900-1700). This is a permanent part time post (30 hours).
- To undertake delegated duties that facilitate clients journey through the service from referral to discharge.
- To assist in the assessment, analysis, implementation and evaluation of programmes of care focused on the target populations under the direction of the registered nurse.
- To take every reasonable opportunity to provide advice on health improvement and health promotion to clients as appropriate.
- To support and signpost clients to mainstream services which will improve the health and well-being.

An Enhanced DBS with Children’s and Adults' Barred List checks will be obtained during pre-employment checks.

This post involves contact with patients and families face to face within a community setting. Therefore it cannot be ruled out that the postholder may come in to contact with people who have COVID-19, although the Trust provides PPE and takes all reasonable steps to minimise risk.

Key results from the job holder

Planning and Organisational Duties
- To adhere to all relevant Trust policies, procedures and protocols, and to contribute to discussions and decisions that involves the team, wider service and organisation.
- To be responsible for the personal input of data to the Community Information System (client activity and outcomes) that will inform the overall service performance reports and health intelligence data for commissioners.
- To receive and participate in clinical supervision, case management under direction from registrant and personal development review from registered nurse.
- To report any financial and physical resource needs to the team lead/operations manager
- To undertake mandatory training as directed by the Trust and any additional training identified as essential to service delivery

To comply with Health and Safety Policies. To ensure prompt recording and reporting of accidents/incidents.
To develop partnerships via pro-actively networking with other professionals and agencies to enhance service delivery.

Communications and Key Working Relationships
- To communicate effectively and appropriately with all clients, members of the Service and other agencies.
- To communicate information to clients, carers, relatives, colleagues and other organisations acknowledging barriers to understanding such as identified language needs.
- To communicate information to clients and relatives with empathy, understanding and reassurance in a non-judgemental way.

Responsibility for Health & Safety
