An exciting opportunity has become available within the Neurology team at Nottingham Children's Hospital, based at Queens Medical Centre.

We are looking for an enthusiastic and motivated individual with a passion for delivering high standards of care to join our team. Excellent communication skills are essential to this role. You must be reliable and able to work flexibly in partnership with and as part of our wider multi-disciplinary team.

**The post holder will be based at**: Nottingham Children’s Hospital, Queens Medical Centre Campus.

To undertake community visits as required, either on their own or to jointly visit alongside the neurology team to support patients in the home or with teaching to nursery/school staff as needed.

To support the co-ordination of new patients identified by genetic testing, arranging appointments, resources and distributing letters to GPs / health visitors and school nursing teams.

To maintain accurate records of all contacts made with patients and families on Trust systems, which should be available to colleagues, senior nursing/medical staff. Records should reflect the present condition and progress of the child.

To organise school training sessions. To book venues, source funding, liaise with schools and the wider community so that training is advertised efficiently.

Undertake clinical observations and procedures (weight/height, blood pressure, urinalysis) within the outpatient setting, and report appropriately, if needed.

Support the organising of patient investigations, reminding families to attend or provide samples.

Communicate effectively with patients, carers/relatives and all multi-professional colleagues regarding all aspects of patient care.

To assist in the paediatric neurology clinics as required.

To refer any patient concerns to the paediatric neurology team.

Have an up to date understanding of safeguarding children and young people.

Liaise with other Trusts to request test results/reports so they can be reviewed by the neurology team.

Supporting the whole Neurology team on joint visits to patients within the hospital and community settings.

Representing the Neurology team on visits to organisations and facilities involved in patient care.

Communicate effectively with patients, carers/relatives and all multi-professional colleagues regarding all aspects of patient care.

To manage own diary and workload based on job responsibilities and the needs of the Neurology service.
