**Job overview**:
We have an exciting opportunity for motivated Health Visitors to join our integrated team. The Health Visitor is the lead Practitioner in the 0-5yrs Oldham Right Start Service delivery model. This integrated model is health led, utilising Children’s Centres as the front door access to services.

The role requires excellent communication and interpersonal skills, the ability to be adaptable, reliable and highly organised. Effective prioritisation skills and caseload management are essential to the role.

**Main duties of the job**:
As the Lead Practitioner you will:
Support early identification of need, to promote the health and wellbeing and safety of the children and their families.

Delegate work to members of the multi-skilled Right Start team, ensuring oversight of the care delivered and supervision to team members.

Have line management responsibility for members of the multi-skilled team, including performance review, supervision and personal development planning.

Development opportunities to deputise for the Team Leader when individuals are ready and to support innovative service developments across the 0-5 Right Start teams.

Our integrated model brings together key professionals from Health, Education, Clinical Psychology and the Special Education Needs and Disability team (SEND). Being based in our children’s centres you will be delivering key services in the heart of the community.

**Detailed job description and main responsibilities**:

- To take named health visitor responsibility as an independent practitioner for continuous responsibility for a designated caseload identified as Universal, Universal Plus and Universal Partnership Plus.
- To develop and deliver healthcare and health promotion using evidence based practice, bringing professional leadership to the wider right start team
- To promote positive health to individuals, families, groups and local communities
- To contribute to the development of best practice and facilitate shared learning to develop services and initiatives that meet local need.
- To develop effective working relationships with key stakeholders in delivering services to enable individuals and families to improve their health and wellbeing
- To appropriately delegate care to members of the multi skilled team ensuring oversight of care delivered and supervision
- Offering evidence based advice, practice, guidance and high level judgement on the management and care of health needs for the whole family.
- To facilitate the delivery of clinic and group activities / sessions as required
- Accountable for own professional practice
- Accepts delegated line management responsibility for members of the multi skilled team which may include performance and development reviews, guidance and management supervision
- In the absence of the team leader will take delegated responsibility for supporting delivery across the team. These duties may include responding to queries from the public, allocation of work, the delegation of duties to wider team members and allocation of team activities

Assessing social circumstances, which may require further support or multi-agency liaison, referral and intervention, identifying risks to the health and well being of children.
