An exciting opportunity has arisen for two new Assistant Practitioner posts to work within Sheffield's Integrated Community Stroke Service (ICSS).

The Sheffield ICSS comprises of a 31 bedded stroke specialist inpatient rehabilitation centre SPARC, and the co located community stroke service. Together these teams support Sheffield stroke survivors to maximise recovery following their stroke and where possible promote empowerment and independence.

This post will form part of a small team of assistant practitioners who will work flexibly across the Sheffield ICSS to provide assessment and rehabilitation and facilitate a seamless transition from hospital to home acting as a key workforce to deliver nationally recommended therapy responsiveness and rehabilitation intensity levels.

The post holders will carry out initial assessments, to assimilate potentially complex medical and social information, resulting in the formulation and delivery of physiotherapy and occupational therapy/rehabilitation treatment plans. The post holder will contribute to the maintenance of governance standards, and supervision of band 3 therapy assistants. The role will entail citywide travel and therefore the post holder must have a full UK driving licence and access to a vehicle.

Duties will include,

Initial assessments to formulate rehabilitation treatment plans.

Use continuous assessment to evaluate treatment outcomes and adjust treatment plans accordingly, appropriately referring to other agencies to meet patients’ needs and promote integration of services.

Work within a legal framework with patients who lack capacity to consent to treatment.

To accept clinical responsibility for patient case load and to organise this effectively and efficiently with regard to clinical priorities and use of time.

Effectively communicate information to patients and carers, who may have barriers to understanding, e.g. dysphasia.

To communicate effectively and work collaboratively, as an independent practitioner within the MDT.

To deliver treatment programmes and reviews independently of a qualified therapist.

Ability to recognise when patients require an assessment by a qualified therapist, and have an understanding and awareness of when to seek involvement of other AHP’s, ie. SLT, Psychology.

To train and supervise senior assistants and assistants in a clinical and organisational capacity, including appraisals.

Equipment ordering.

Patient motivation and empowerment.

Work within the governance framework.

To undertake frequent therapeutic handling and moving of patients, with or without aids, and equipment utilising moderate physical effort within clinical and non-clinical settings.

Deliver aspects of stroke prevention and education to patients.
