**Job Summary**

This Clinical Research Fellowship provides an exciting opportunity to pursue high quality, important and impactful research through examination of equity of access to the liver transplant waiting list and to transplantation in the UK, based on socioeconomic status; impact on registration, organ offering, allocation and transplant outcomes.

Liver disease is associated with deprivation and disproportionately affects those experiencing health inequalities who have higher mortality also. Little is known about whether these factors are fairly represented amongst those accessing the liver transplant waiting list and whether there is socioeconomic inequality in offering and allocation of donor livers within and out with the national liver offering scheme. We aim to establish whether there are inequities within these selection processes and whether they were exacerbated by the COVID pandemic. There will also be the opportunity to assess the impact of socioeconomic status on outcomes after liver transplantation and to consider pilot evaluations of transplant for new indications.

The post will provide a three-year period of full-time research training and the successful applicant is expected to register for a higher degree (PhD or equivalent).

Working under the direction of Professor Douglas Thorburn and Professor Julia Verne, the appointee will work closely with NHSBT Statistics and Clinical Research team in Bristol and with other senior colleagues at all UK liver transplant centres.

**Main duties of the job**

Through analysis of datasets held within NHSBT and NHS England, the post-holder will work to establish if:

- there is socioeconomic inequality in the selection of patients for liver transplant in the UK.
- there was any impact on socioeconomic status of patients listed during the coronavirus pandemic and for those registered with new cancer indications for liver transplant.
- there is inequality of access to liver offers and subsequently transplantation for patients of differing socioeconomic status and those with protected characteristics once they have been activated on the transplant waiting list.
- socioeconomic status impacts on patient and graft outcomes after transplantation.

SPECIFIC TASKS FOR THE CLINICAL RESEARCH FELLOW:

- Conduct a systematic literature review on health inequalities, liver disease and organ transplantation
- Work across datasets from NHSBT and UKHSA, to compare patient populations
- Retrospective analysis of data already captured on the NHSBT transplant database, to determine the existence of health inequalities.
- Prospective evaluation of access to transplant for patients within new pilot service evaluations, including new cancer indications.

It is expected that the work would lead to a PhD, high impact publications, oral presentations at national and international conferences and the development of skills to facilitate a future career in organ transplantation and/or public health research.

The post-holder will be employed by NHSBT and will work closely with NHSBT and hold an honorary clinical contract with the Sheila Sherlock Liver Centre at the Royal Free Hospital, London.

**Qualifications and Training**
- MBBS or equivalent medical qualification.
- Full GMC registration with a licence to practise.
- Evidence of good progress in post-graduate clinical training.
- MRCS or MRCP (or equivalent)
- Completion of foundation programme training or equivalent
- Additional post-graduate training.
